The key task will be to provide information to generally older clients and their families about choices in care provision and to advise on all relevant aspects of their care.
In support of this role you will:
Conduct Consultations by telephone or in person
• Listen to the Client’s needs and wishes
• Demonstrate an understanding of all aspects of care provision
• Advise the Client about their care options
Research Care Options
• Liaise with other health care professionals when permission has been given to do so.
• Identify and contact relevant care providers to discuss individual care needs and personal requirements
• Consider private, statutory and voluntary organizations
• Select most suitable care providers for Client’s report
Report to Client
• Provide detailed, accurate and relevant information about Care Providers
• Provide supplementary factsheets and information as appropriate to the individual’s circumstances
• Address specific queries or concerns raised at the time of the Consultation
• Provide ongoing support as required by the Client until a satisfactory decision has been made
In certain circumstances there will be the need to carry out this work within a defined deadline agreed with the client at the time of the Consultation.
In addition to this key task, you may be asked to:
• conduct reviews of “out of area” Local Authority Care Home placements
• conduct an independent nursing assessment of the client’s health and care needs.
• assess Care Homes
You will be accountable to the Care Advice Manager or Nurse Manager for the purpose of this work. You will be fully supported and trained for this role.
OUR CLIENT IS LOOKING FOR SOMEONE WITH A NURSING, SOCIAL WORK OR OCCUPATIONAL THERAPY BACKGROUND WHO HAS HAD RECENT AND REGULAR CONTACT WITH OLDER PEOPLE.
